Welcome to Aspire Social Enterprise Counselling Service. Our terms of service and your rights and responsibilities in relation to information security, access, and confidentiality as well as your obligations regarding fees and cancellations are provided below:
CODES OF PRACTICE:
As a registered counsellor with the Australian Counselling Association (ACA) your counsellor adheres to the associations’ ethical codes and confidentiality principles as well as to the legal requirements of the Information Privacy Act 2009 (QLD).
USE AND DISCLOSURE OF INFORMATION:
In some situations, disclosure of personal or health information may be necessary where there is a serious and imminent threat to an individual’s life, health or safety, or a threat to public health or safety. Disclosure is required if withholding certain health information could affect the life, health and safety of a genetic relative.
These situations are:
- When subpoenaed by court, or
- When required by law (e.g. Mandatory Reporting for child protection, unlawful activity), or
- When failure to disclose information would place you or another person at risk of serious harm, or
- When your prior approval has been gained for authority to release information to a nominated person.
Health research using de-identified data may be undertaken by this practice, by funding bodies or by this practice’s technology providers, which you consent to as a patient of this practice.
INFORMATION SECURITY AND ACCESS:
Personal information will only be collected when it is lawful to do so and when directly related to the counsellor role and the activity of managing the service. Information will only be collected from you directly when it is relevant. Your clinical records are stored electronically in your client file on Halaxy, which you consent to as a client of this service. You have a right to access your records with reasonable notice.
We are required to store your personal information for 7 years after ceasing engagement with your counsellor.
CONFIRMING APPOINTMENTS:
We will endeavour to contact you via SMS or email to confirm appointments; however, it remains your responsibility to be aware of your scheduled appointments.
TIME AND PUNCTUALITY:
A consultation will usually last 60mins. If you are late, your consultation will usually still finish at the scheduled time, to be fair to your counsellor and the clients with appointments after yours.
CANCELLATION POLICY:
If you need to cancel or postpone an appointment, we require at least 24hrs notice. Cancellations less than 24 hrs. notice will incur a fee of $45.
SERVICE/CONSULT FEES:
Fees are payable at the time of your appointment. Payments accepted include cash or debit/credit card. Fees are on our website.
